Methodology
Test may be performed at employer’s laboratory of choice. If laboratory is not specified, specimen will be sent to Quest Diagnostics Laboratory in Lenexa, KS for testing.

Specimen Requirements
1. Place blue dye into toilet bowl.
2. Instruct patient to collect a urine specimen, and give it to the appropriate person prior to flushing toilet or washing hands.
3. Submit 50 mL (minimum volume: 30 mL) from a random urine collection using Drug Screen Kit. No preservative.
4. Split specimen if required, and affix chain-of-custody tape across top of urine container. No STATS are accepted for this procedure. No specimen substitutions.
5. Place specimen and completed chain-of-custody form in a sealed bag.
6. Send specimen at ambient temperature.
