Methodology
Test may be performed at employer’s laboratory of choice. If laboratory is not specified, specimen will be sent to Quest Diagnostics Laboratory in Lenexa, KS for testing.

Specimen Requirements
1. Prep patient’s arm with Betadine®.
2. Draw blood in a grey-top (potassium oxalate/sodium fluoride) tube(s).
3. Affix chain-of-custody tape across top of tube(s), and send 5 mL of (potassium oxalate/sodium fluoride) whole blood.
4. Place tube(s) and completed chain-of-custody form in a sealed bag.
5. Send specimen at ambient temperature.
